New Students
SAS welcomes all current undergraduate and graduate students at Ontario Tech with permanent, prolonged or persistent and temporary disabilities. We can also support students who are managing acute injuries related to athletics or accidents. This includes short or long term symptom managment of limb fractures, concussions, chronic pain or other medical issues. We look forward to connecting with you and providing support on your academic journey.

Please note that providing incomplete information will result in the delay or inability of our office to process your documentation.
Individual Education Plans (IEPs) and accommodation plans from previous years of study are helpful tools in accommodation planning, however, they are not considered confirmation of disability. Confirmation of disability will be required beyond these documents to receive accommodations at the university.
Please note: Previous accommodation plans from other institutions will not automatically transfer to Ontario Tech. You will need to request your documentation from previous schools. Schools do not share this information with one another.

Complete the Intake Form
Collect and/or complete your disability documentation. If you are unsure about documentation please discuss existing documentation and options for pursuing updated documentation in your intake appointment.
Accepted forms of Disability Documentation:
- Autism Spectrum Disorder (ASD) Assessment.
- Neuropsychological (NEURO) Assessment.
- OSAP Disability Verification form (2023-2024).
- Psychoeducational Assessment (PEA).
- SAS Disability Documentation form.

This is a great option for students who may be going into experiential learning programs during their time at the university.
Documentation must:
- Confirm the disability is present, whether temporary or permanent.
- Identify functional limitations and disability-related needs.
- Indicate how the disability impacts or impairs academic functioning.
- Must adhere to the scope of practice of regulated health professionals in Ontario.

If you have any questions or concerns about sufficient documentation or experience any accessibility isssues when attempting to complete the forms let our staff know and you can complete your forms with support when you attend your intake appointment.
Once we have recieved your intake, consent and disabiltiy documentation forms, studentaccessibility@ontariotechu.ca will email you to coordinate your appointment. Appointments are usually booked for two hours so please ensure you are available for the full duration.
